Four Psychosocial Factors when Working with Refugees and Immigrant Children

Psychosocial factors that must be considered in order to understand the psychosocial context of the refugee and immigrant client are: post-traumatic stress disorder, depression, cultural barriers, and discrimination. Many people who migrate to the United States are fleeing from a tumultuous situation in their country in hopes of better opportunities that may be presented in the United States. Those people may suffer from post-traumatic stress disorder due to a war in their country for example, or even just the experiences faced when crossing the border. Those who successfully cross the border may also suffer from depression because they left their family, their friends, their job, and their familiar neighborhood in order to live in an entirely new country. Several of these experience cultural barriers which make it difficult for them to communicate with others and understand the culture in the United States which can subject them to discrimination.

According to Ronstrom (1989) state that children in Central American children in America suffer from anxiety, demand for attention, poor self-image, regressive behavior, aggression and many other psychological issues such as secondary traumatization where children are separated from their mother (as cited in Masser, 1992). This trauma experienced by a young child can be detrimental for their psychosocial functioning and this child may be prone to post traumatic stress disorder at such a young age. In fact, a strong correlation in psychosocial functioning in adult life was with the degree of trauma experienced in postwar separation from the parents (Masser, 1992).

Psychosocial Intervention to Address Post Traumatic Stress Disorder in Children

The social worker can implement trauma focused psychotherapies to intervene such as traumatic incident reduction. According to SAMHSA’s National Registry of Evidence-based Programs and Practices (NREPP)(n.d.), traumatic incident reduction (TIR) is a brief memory-based intervention used for all populations which is designed to “resolve symptoms of posttraumatic stress disorder (PTSD), depression, anxiety, and low expectancy of success by integrating dissociated cognitive and emotional aspects of traumatic memory” (para 1).

Measures Used to Evaluate the Effectiveness of the Intervention

According to Descilo, Greenwald, Schmitt, and Reslan (2010), the assessments used to evaluate the effectiveness of TIR are the Post Traumatic Stress Disorder (PTSD) Check List (CL) which is a 17-item self-administered scale that ranges from 1 (not at all) to 5 (extremely); the score is added and a score greater than 30 is most likely PTSD. Another assessment used is the Center for Epidemiologic Studies Depression Scale (CES-D) which is a 20-item self-report depression scale measuring 0 to 3 which a score of 16 or higher indicating probable moderate to high levels of depression (Descilo et. al., 2010). The assessments can be given before treatment (pre-test), during treatment (mid-test), and after treatment (post-test) to indicate whether the results from the PTSD PCL and the CES-D test have improved, stayed the same, or not improved.

Advantages of Using the Psychosocial Framework in Social Work

One advantage of using the psychosocial framework in social work is that we can focus on the client’s strengths as an empowerment tool. Turner (2017), states that “the strengths perspective capitalizes on very empowering notions that social work had in its more philosophical epoch and was further undertaken in the 1960s, as a result, in part of minority movements in the United States and Canada” (p. 416). Providing that safe space for the child to explore their strengths and resiliency can empower them and hopefully minimize any negative thought processes.

Four psychosocial factors that should be taken in to account when working with my selected group is depression and hostility which are psychologically related and hopelessness and stress which is socially related. Those individuals that are not readily accepted for their lifestyle may experience a high level of stress, anxiety and depression. This may lead them to turn to alcohol to try and cope with what they are dealing with. Alcohol use may leave the individual feeling hopeless and regretful for being honest about their sexual identity (Santos & VanDaalen n.d.).

Identify and explain a psychosocial intervention that addresses one of the four specific psychosocial factors you described above.

One on one time with a licensed professional therapist for is a way of addressing the psychosocial factor that I mentioned regarding depression. When a person is not interacting with society well, psychosocial interventions can be used to help guide that person back into a healthy state of being. The way this is typically done is by the use of non-medicinal means. This is done to alter the way a person’s behaviors and relationships are within society. This is done to reduce the impact of the person’s disorder or condition. The main key to psychosocial interventions is that it does not use pharmaceutical assistance in the attempt to change a person’s behaviors toward a healthier interaction within society.

Identify 1 to 2 outcomes you might measure if you were to evaluate the effectiveness of the intervention.

An outcome that might be measured is the client’s ability to change negative thoughts. This could be a person that feels that she is not loved and shunned because she is a lesbian. As a social worker I can encourage the person to appreciate who she is and surround herself with other positive individuals who have been through what she may be going through. and accept the way they are. The effectiveness of the intervention by working with a licensed professional that specializes in this will help to enhance the efforts put for by the social worker as well.

Evaluate one advantage of using a psychosocial framework in social work practice with the group you chose.

One advantage of a social worker using a psychosocial framework in social work practice when working with the lesbian population, is that the goal is to restore, maintain, and enhance their personal and social functioning (Turner, 2017). It’s not to ignore their pain and suffering but enables them to make it under such tough times and builds on their strengths as a member of the LGTB population. This is especially important as a social worker that may have to manage a case load that includes those of the LGTB population. An important feature of partnering case management with social role theory in a way that enhances the delivery of case management services to meet the needs of those we will work with (Blakely & Dziadosz, 2008).
