Explanation of Content/Theme of Paper Using the ‘transtheoretical (behavior change model)’, select a health-related behavior you wish to change with a frame for the plan as six months. The behavior should be exact. For example, if you want to improve ‘physical wellness’, select a small detail (body fat percentage, cardiovascular health or specific body part) to change. Use all six specific steps of the model as headers for each section: Pre-contemplation, Contemplation, Preparation, Action, Maintenance, Termination. A minimum of TWO (2) sources must be used for research. DO NOT use any source before 2015. All sources will be listed on a separate page following the end of the paper. Pre-contemplation: Specifically explain how the problem negatively affects your life. Include why you cannot currently fix the problem. What is holding you back? What did you learn about the issue you want to change? What mistaken beliefs (if any) did you have before the research? Based on research, what did you learn specifically that might work and other things to avoid? Draw conclusions about how the research will guide your attitude/mindset when preparing the plan. Study varying demographics in your world and beyond, including a global perspective. If perchance the percentage of people your age having the same issue is lower in a different part of the world, research why and incorporate it into your personal plan if appropriate. This should be at least four to five substantive paragraphs. It might be the longest section of the paper. Action: Describe every small detail of the plan from the day you begin to the day you end. Include backup plans if there is a problem along the way. How will you keep track of progress? Who will assist you with motivation? How will you alter the plan if your life suddenly changes instead of quitting? This should be at least three to four substantive paragraphs. Maintenance: Describe what happens after achieving your goal. What behaviors will ensure you do not backtrack/relapse and return to the bad behavior?
